Contest ban template

A good idea that would help in the future is a contest ban template. 
Something that looks like this:
Ingame name:
Steam ID:
Reason given(on ban list):
Banning Admin(on ban list):
Reason for unbanning:

This should help us in the future to process bans and stuff.
Here is another to report an admin
Admin ingame name:
Reason for reporting:

And another one for player reporting:
Player ingame name:
Player steam ID:
Reason:
This should help because bans and reports will be a bit more organized and less harder to find stuff in.

I thought about that also but I haven't been able to get it to auto set a post template.

That would be cool to auto set it but you don't have to. Just make a sticky that says "Contesting Ban? Read first." or something like that but it would be awesome if it does have an auto template.
