First off your unban request will not even be considered unless you follow the unban request form Here.
Now, you're computer is your own problem and I doubt someone would be able to just hop on your computer, come to a server you frequently play on, and just know how to micspam. AND it is your own responsibility to manage your own account and what happens on it.

As rusty said, no form = no unban. U provided absolutely no info.

SteamID is the number/text beside your name when you type "status" in your console while in game. STEAM_0:0:000000000
I do not see the name you've given on the ban list for the deathrun or the cheat server.
Do you remember if you were using a different name at the time? If so, provide that name so that we can find out if you are permbanned or just temporarily banned.
